Social Prescribing Service
Worcester City GP Practices have recently been supported by the Clinical Commissioning Group and the County Council to pilot a social prescribing service for patients registered to one of the 10 Worcester City GP Practices.
Social prescribing is a way of connecting health and social care users to sources of support, information and advice to improve their wellbeing, tackle social isolation, and prevent illness.
What is Social Prescribing?
Social prescribing is designed to support people with a wide range of social, emotional or practical needs. It is a way of enabling GPs, health and social care professionals, the voluntary sector and other partners such as pharmacies, to refer people to a social prescribing advisor.
The scheme is evolving in Worcester and is currently available in all GP practices.
How does it work?
Individuals are referred to a social prescriber who will work with them to discuss their health and well-being. Together they will come up with a plan of action that refers them to community activities that can make them feel happier and healthier.

Worcester City GP Practices now have access to two Social Prescribers who will be responsible for delivering a person-centred holistic service, supporting clients to improve their health and wellbeing by signposting to and connecting with local services and assets in their community.
The Social Prescribers will support the client over a time limited period to achieve the goals outlined in the Social Prescription, whilst encouraging and supporting clients to self-manage their condition(s) and develop healthier behaviours and lifestyles.
