Register as a New Patient

Join us as a Patient at Spring Gardens Medical Practice.

Our Surgery Area

Spring Gardens Medical Practice is proud to support patients across Worcester City.

We have recently had a change to our practice boundary to exclude patients from across the river. This mostly affects patients with the WR2 area of the City.

Severn Valley and St John’s medical practitioners will be able to accept your registration.

If you live within our area, you can ask to be registered with the Surgery and, providing the Surgery list is not closed, you may be registered as a patient with the Surgery.

We look forward to hearing from you!

Joining the Surgery

To register, we ask that all patients use our online registration applications. If you need paper forms, feel free to visit the Surgery and collect forms from reception. If you have any problems completing the digital forms, please call the Surgery on 01905 744400.

There are some restrictions on registration for visitors from abroad. For more information on this visit NHS For Visitors From Abroad

Online Registration

Click the button below to register with our practice online.

Please note registration can take around 45 minutes, as we ask for a range of health information from you - so we recommend starting the process when you have the time and space to do so.

Temporary Residents

If you are not registered with a GP but need to see one, you can get emergency treatment at any GP surgery.

If you need treatment for more than 14 days, you will need to register with that GP surgery as a temporary or permanent patient.

You can register as a temporary resident if you plan to live near the GP surgery for up to 3 months. After 3 months you will have to apply to register with that surgery as a permanent resident.

You can register temporarily with a GP surgery while away from home for work, study or on holiday. You’ll remain registered with your permanent GP surgery.

If you move out of the Practice area, you will need to register with a new Practice as soon as you move. If you need help finding a new Practice, contact the local NHS Health Board.

What information do I need?

Try to have the following information with you when you see a temporary GP for the first time:

  • details of any medical conditions you have

  • details of medical conditions you’ve had in the past

  • the name of any medicines you’re currently taking

  • details of anything you’re allergic to

  • contact details for your permanent or previous GP surgery

  • your NHS number

You’ll also need to complete a Temporary Resident Form (GMS3). You can download this by using the button below.
